What causes heel pain in adults and children?

Heel pain is a common problem in both adults and children, but they are generally commonly caused by two totally different unrelated entities. In adults, the most common cause of heel pain is a condition known as plantar fasciitis and the most common condition in children is a condition known as calcaneal apophysitis or Sever’s disease. One is an overuse problem of the plantar fascia and the other is an inflammation in the growing plate at the back of the heel bone.
The plantar fascia is a strong ligament that supports the arch of the foot, so anything such as higher activity levels, tight calf muscles or being overweight which puts more pressure on the arch is going to increase the risk for plantar fasciitis in adults. The characteristic symptom of this is pain under the heel that is significantly worse in those first few steps after rest, especially getting out of bed in the morning. This pain does tend to improve after those first few steps but does tend to be progressive. The best way to deal with plantar fasciitis is to lose weight, do lots of stretching for the calf muscles and use foot orthotics to reduce the force that gets applied to the plantar fascia. Ice can be used on it and anti-inflammatory drugs could also be used if the pain is limiting daily activities. There are other modalities such as injection therapy and shock wave therapy that can also be used to help facilitate healing. In rare cases surgery might in needed.
At the back of the heel bone in children is a growth plate where the growth of the bone occurs at. These growing areas of bone are prone to being damaged if the child is overweight or very active. Severs disease at the back of the heel bone is very common in children in the late pre-teen years. The growth plate merges with the rest of the heel bone in about the mid-teenage years, so Sever disease is always self-limiting and will go away on its own when this occurs. Just because it is self-limiting does not mean that it should not be treated as it can be quite painful and distressing for the child who has it. The pain is typically at the back of the heel bone, especially if you squeeze it from the sides and is worse with increased levels of activity. Occasionally, the pain from the condition can cause the child to limp. The first approach to treatment is education as to the exact nature of the problem and to manage how active the child is. Managing this is challenging as they want to be active both in sport and at school, but activities need to be restricted to what they can tolerate. Activity levels can be increased as it improves. Ice can be used after sport if it is painful. Cushioned heel pads are also particularly helpful at reducing the forces on the heel bone. Foot orthotics can also be helpful in many cases.

काठमाडौं । नेकपा (एमाले)का अध्यक्ष केपी शर्मा ओलीले सक्रिय भएर काममा लाग्न निर्देशन दिएका छन् । आज पार्टी केन्द्रीय कार्यालयमा आयोजित पार्टी प्रवेश कार्यक्रममा उनले अहिले पनि आफू १८ घण्टा काम गर्ने गरेको सुनाए । नेताकार्यकर्ताहरूलाई पनि १४ घटासम्म काम गर्न उनको आग्रह छ ।
“अहिले पनि म १८ घण्टा काम गर्छु । प्राय साथीहरू त भन्दा कमै उमेरका हुनुहुन्छ । १८ घण्टा म काम गर्छु भने १८ घण्टा नगर्नुस् कमसेकम १४ घण्टा त गर्नुस्,” ओलीले भने, “१२ घण्टा गर्नुस्, १० घण्टा गर्नुस् त्यो सक्नुहुन्न भने ८ घण्टा त गर्नुस् ।८ घण्टा एउटाले गर्दा धेरै काम हुन्छ । सक्रिय बनौँ ।”

उनले पार्टीविरुद्ध कसैले उचालेमा त्यसमा नलाग्न आग्रह गरे । उनले उमेदवार नभएपनि जिताउनका लागि लाग्न निर्देशन दिए ।“उमेदवार हुने जित्नका लागि काम गर्नुस् । उमेदवार नपाउनेहरूलाई उमेदवारलाई जिताउन लाग्नुस् । एकजुट भए जहाँ पनि जित्न सकिन्छ,” ओलीले भने ।उनले अब निर्वाचन लागेको भन्दै विनम्र र शालीन ढंगले प्रस्तुत हुन पनि आग्रह गरे । टोलटोलमा संगठन र घरघरमा सदस्य बनाउनुपर्ने उनको भनाइ छ ।

काठमाडौ, भदौ ७ कांग्रेस नेता एवं सांसद डा. मीनेन्द्र रिजालले नेकपा एमाले अध्यक्ष केपी शर्मा ओलीलाई उहाँकै निजी निवास बालकोट पुगेर भेट गर्नुभएको छ । रिजालले सोमबार झन्डै सवा घन्टा सौहार्दपूर्ण भेटघाट गर्नुभएको हो । भेटमा उहाँहरूबीच राजनीतिक विषयमा अन्तरंग कुराकानी भएको नेता रिजालले बताउनुभयो ।
सत्ता गठबन्धनसँग सकारात्मक सम्बन्ध नरहेका प्रमुख विपक्षी नेता ओलीसँगको भेटका लागि नेता रिजालले नै अग्रसरता लिनुभएको थियो ।
‘हामीबीच खुला रूपमा सन्तोषजनक कुराकानी भयो,’ रिजालले भन्नुभयो , ‘म मेरो पार्टीको नेतृत्वलाई जानकारी दिनुपर्ने ठाउँमा यसबारे बताउनेछु तर सार्वजनिक रूपमा यसबारे केही बताउन उचित हुन्न।

Nepali Congress to mourn Pradip Giri’s death for 13 days !!

Kathmandu, August 21

The ruling Nepali Congress party has decided to mourn its intellectual leader Pradip Giri’s death for 13 days.
An emergency meeting of the party’s central working committee held on Sunday morning decided to hoist the party flags at half mast for the first three days as a sign of mourning. The party’s key offices across the country will have a condolence book to let people express their final homage to the popularly known socialist thinker.

Likewise, the party will host condolence assemblies on the 13th day of his death.

Until then, Nepali Congress will not hold any formal event except for condolences and mourning.

Pradip Giri a sitting member of the House of Representatives, died on Saturday.

Impacts of heat on sleep !!

Neurobiologists discovered that fruit flies are pre-programmed to take a nap in the middle of the day. The new research examines a ‘thermometer’ circuit for hot temperatures.

Temperature affects the span of human behavior, from eating and activity levels to sleep-wake cycles. We may have a harder time sleeping in the summer and be slow to get out of bed on colder mornings. But the link between sensory neurons and neurons that control this cycle are not understood completely.

Northwestern University neurobiologists have found a few clues about what’s happening. In a new study, published today (Aug. 17) in the journal Current Biology, researchers found that fruit flies are pre-programmed to take a nap in the middle of the day. A follow-up to their 2020 Biology paper that identified a brain thermometer only active in cold weather, the new paper explores a similar “thermometer” circuit for hot temperatures.

“Changes in temperature have a strong effect on behavior in both humans and animals, and offer animals a cue that is time to adapt to the changing seasons,” said Marco Gallio, associate professor of neurobiology in the Weinberg College of Arts and Sciences. “The effect of temperature on sleep can be quite extreme, with some animals deciding to sleep off an entire season — think of a hibernating bear — but the specific brain circuits that mediate the interaction between temperature and sleep centers remain largely unmapped.”

Gallio led the study and said fruit flies are a particularly good model to study big questions like “why do we sleep,” and “what does sleep do for the brain” because they don’t attempt to disrupt instinct in the same way humans do when we pull all-nighters, for example. They also allow researchers to study the influence of external cues like light and temperature on cellular pathways.

CELLS THAT STAY ON LONGER
The paper is the first to identify “absolute heat” receptors in fly head, which respond to temperatures above about 77 degrees Fahrenheit — the fly’s favorite temperature. As it turns out, the common laboratory fruit fly (Drosophila) has colonized nearly the entire planet by forming a close association with humans. Not surprisingly, its favorite temperature also matches that of many humans.

Just as they expected based on the results of their previous paper on cold temperature, researchers found that brain neurons receiving information about heat are part of the broader system that regulates sleep. When the hot circuit, which runs parallel to the cold circuit, is active, the target cells that promote midday sleep stay on longer. This results in an increase in midday sleep that keeps flies away from the hottest part of the day.

The study was enabled by a 10-year initiative that produced the first completed map of neural connections in an animal (a fly), called the connectome. With the connectome, researchers have access to a computer system that tells them all possible brain connections for each of the fly’s ~100,000 brain cells. However, even with this extremely detailed road map, researchers still need to figure out how information in the brain goes from point A to B. This paper helps fill that gap.

The different circuits for hot versus cold temperatures make sense to Gallio because “hot and cold temperatures can have quite different effects on physiology and behavior,” he said. This separation may also reflect evolutionary processes based on heat and cold cycles of the Earth. For example, the possibility that brain centers for sleep may be directly targeted in humans by a specific sensory circuit is now open to be investigated based on this work.

NEXT STEPS
Next, Gallio’s team hopes to figure out the common targets of the cold and hot circuit, to discover how each can influence sleep.

“We identified one neuron that could be a site of integration for the effects of hot and cold temperatures on sleep and activity in Drosophila,” said Michael Alpert, the paper’s first author and a post-doctoral researcher in the Gallio lab. “This would be the start of interesting follow-up studies.”

Gallio added that the team is interested in looking at the long-term effects of temperature on behavior and physiology to understand the impact of global warming, looking at how adaptable species are to change.

“People may choose to take an afternoon nap on a hot day, and in some parts of the world this is a cultural norm, but what do you choose and what is programmed into you?” Gallio said. “Of course, it’s not culture in flies, so there actually might be a very strong underlying biological mechanism that is overlooked in humans.”
